Thank you for your interest in Lake Hazel Labradoodles.  The purpose of this application is to insure the best Owner/puppy match as possible.  To best achieve this we ask you to be as complete as possible in your responses.  In order to reserve a puppy, this completed application form must be received along with a non-refundable deposit (which will be applied to the purchase price of the puppy) payable by cash, Paypal or money orders made out to Adam Rogers. (No Personal Checks)  An application received without a deposit will not reserve a puppy.  We reserve the right to decline any application.  If your application is declined you will receive a full refund of your deposit.
